
![]() A Quality Care Management Program for Asthma Nearly twenty million Americans suffer from asthma, and unlike many diseases, asthma is not restricted to any one age group.Through AsthmaCare, patients can gain a deeper understanding and the ability to take control of this chronic disease. How AsthmaCare Works: AsthmaCare is designed to help patients follow their prescribed treatment plan by providing education about their disease, medications used, potential complications and healthy lifestyle choices. Ultimately, it is intended to improve the overall patient-physician relationship. 1. Through monthly claims analysis, our staff identifies and contacts members with asthma about the program. Members may then choose to either opt out or continue to be "enrolled"in the program. 2. Once enrolled, members will receive educational materials on a quarterly basis. This information includes topics such as asthma triggers, physiological aspects, medications used to treat asthma, developing and following a personalized action plan, self-help tools, free peak flow meter and spacer offers, and more... 3. Program materials are sent to the member's primary care physician to notify them of the member's enrollment in AsthmaCare. 4. Pharmacy and claims data is monitored to identify members at higher health risk. These members will receive all of the above interventions in addition to individualized care management by a Registered Nurse. This may include a phone call from the RN to discuss proper medication usage, assistance with scheduling appointments, and reinforcement of educational needs. 5. Enrolled members are offered support through an established toll-free phone number for clinical and behavioral support as needed. The AsthmaCare Program will help members identified with asthma take proactive care for improving their individual health to avoid any high-cost medical events and claim costs. |
